{"id":89,"date":"2021-12-09T15:38:33","date_gmt":"2021-12-09T20:38:33","guid":{"rendered":"https:\/\/mitaxapp.com\/D15website\/?page_id=89"},"modified":"2026-01-24T21:42:35","modified_gmt":"2026-01-25T02:42:35","slug":"information-and-documents-to-bring","status":"publish","type":"page","link":"https:\/\/mitaxapp.com\/D15website\/information-and-documents-to-bring\/","title":{"rendered":"Information You Need to Bring"},"content":{"rendered":"\r\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\r\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis: 100%; padding: 25px;\">\r\n<div class=\"wp-block-group has-foreground-color has-secondary-background-color has-text-color has-background\"><div class=\"wp-block-group__inner-container is-layout-flow wp-block-group-is-layout-flow\">\r\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-9d6595d7 wp-block-columns-is-layout-flex\">\r\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis: 85%;\">\r\n<p><a name=\"info-to-bring\"><\/a><strong><u>DOCUMENTS TO BRING <br \/><\/u><\/strong><\/p>\r\n<p>Please bring ALL the documents listed below that you may have received or risk multiple trips to get your tax return completed:<\/p>\r\n<ol>\r\n<li><strong>Photo ID \u2013 <\/strong>Driver\u2019s license or another government-issued photo ID.\u00a0<\/li>\r\n<li><strong>Social Security Card<\/strong> \u2013 For you, your spouse, children and ALL dependents.<\/li>\r\n<li><strong>1095-A Form<\/strong> &#8211; For health insurance through the Marketplace &#8211; Affordable Care Act (Obamacare)<\/li>\r\n<li><strong>Prior year Tax Return Copie<\/strong>s &#8211; We\u00a0<strong>CANNOT<\/strong> \u201clook up\u201d information for you.\u00a0 \u00a0You will need to contact IRS directly yourself<\/li>\r\n<li><strong>Social Security Statement \u2013 <\/strong>Form SSA-1099.<\/li>\r\n<li><strong>Documents for all income and payments you received or made:<\/strong><\/li>\r\n<\/ol>\r\n<ul>\r\n<li style=\"list-style-type: none;\">\r\n<ul>\r\n<li>SSI Statement \u2013 Disability payments if you are disabled.<\/li>\r\n<li>W-2\u2019s from all employers.<\/li>\r\n<li>Gambling winnings (W-2G)<\/li>\r\n<li>Pension, IRS and\/or annuity statements (1099-R) \u2013 If any of these statements are missing, the IRS will impose interest and penalties if they are not reported.<\/li>\r\n<li>Unemployment income or tax refunds (1099-G).<\/li>\r\n<li>Any other 1099 Forms (there are many types) \u2013 from interest, dividends, part-time work, stock sales, etc.<\/li>\r\n<li>Self-employment income or any other income, <span style=\"text-decoration: underline;\">even if undocumented or part-time<\/span>.<\/li>\r\n<li>Letter 6419 Advanced Child Tax Credit payments received<\/li>\r\n<\/ul>\r\n<\/li>\r\n<\/ul>\r\n<p><strong>IF YOU PLAN TO ITEMIZE DEDUCTIONS,<\/strong> bring<\/p>\r\n<ul>\r\n<li>List and total of all medical expenses not paid by insurance \u2013 only the part that exceeds 7.5% of your Adjusted Gross Income is deductible.<\/li>\r\n<li>Property tax and other taxes paid (auto registration, sales tax on auto, boat or other large purchases)<\/li>\r\n<li>Total charitable contributions paid in cash or check, and an itemized list.<\/li>\r\n<li>Receipts of donated items.<\/li>\r\n<li>Dependent care provider\u2019s name, address and SSN or ID Number and receipts showing total amount paid.<\/li>\r\n<li>List and total for other allowed deduction.<\/li>\r\n<\/ul>\r\n<p><strong>IF YOU WANT THE MI \u201cHOMESTEAD PROPERTY TAX CREDIT\u201d OR RENTER\u2019S CREDIT<\/strong>, bring<\/p>\r\n<ul>\r\n<li>Property tax bills \u2013 showing assessed value and receipts showing taxes <u><\/u><\/li>\r\n<li>Rent receipts and total rent paid for the year.<\/li>\r\n<li>Name and address of landlord.<\/li>\r\n<li>If you live in co-op housing or partially subsidized housing, bring your letter from the co-op showing your taxable value and your rental agreement.<\/li>\r\n<\/ul>\r\n<p><strong>IF YOU WANT THE MICHIGAN HOME HEATING CREDIT,<\/strong> bring<\/p>\r\n<ul>\r\n<li>Total heating cost for the year \u2013 usually shown on the most recent December or January utility bill.<\/li>\r\n<\/ul>\r\n<p><strong>OTHER TAX FORMS AND LETTERS YOU MAY NEED TO BRING:<\/strong><\/p>\r\n<ul>\r\n<li>Identity theft PIN number and letter from IRS \u2013 if you have had a tax ID theft in the past.<\/li>\r\n<li>Education scholarship form \u2013 1098-T and other documents showing expenses paid.<\/li>\r\n<li>IRS or MI tax letters you received in the last year.<\/li>\r\n<li>Estimated tax paid \u2013 if you made payments yourself.<\/li>\r\n<\/ul>\r\n<p><strong>IF YOU WANT REFUNDS DIRECTLY DEPOSITED INTO YOUR BANK ACCOUNT,<\/strong> bring<\/p>\r\n<ul>\r\n<li>Blank check showing bank routing and account numbers.
